What if I self-pay or am out-of-network?

We can provide a superbill upon request. A superbill is a detailed receipt containing all of the information your insurance company would require for possible reimbursement. This will be dependent on your individual out-of-network insurance benefits. We’re happy to verify your benefits prior to your initial appointment and to guide you in the superbill submission process.

Do I need a script from a physician to get reimbursed?

No, Illinois is a direct access state. If you have a PPO plan, you do not need a referral or script to access physical therapy services.

At this time, all appointments are scheduled with 24-hour notice, but same day appointments may be available, just give us a call at (630) 474-1287.
